A Review of the Latest Information Technology and Chronic Care Model in Chronic Kidney Disease

Many literatures support the use of information technology (IT) in chronic disease management to improve self-efficacy and health outcomes. Innovative solutions embedded in health IT proven to assist providers to communicate, engage, educate and empower patients with chronic illness such as diabetes, hypertension, asthma, arthritis and cancer. However, there are limited studies exploring on the effects and effectiveness of health IT applied in chronic kidney disease (CKD) care. This review explored the existing chronic care model and it potential to integrate with IT innovations to enhance the key elements in self-management. An enhanced framework called the Informatics Self-management is designed to promote the implementation of nursing informatics in supporting CKD self-management that is focusing on patient-centredness.
